Yes, our optometrists can complete vision forms required for driving, employment, or specialized professions.

Please bring the required paperwork to your appointment. Contact the clinic directly in advance, as some forms may require additional testing and may involve an extra fee.

Applies to same-day purchases only at our retail optical partner and may be used toward glasses or contact lenses. Excludes eye exams and professional fees.  

Our clinic is located inside West Edmonton Mall, within the For Eyes by Clearly store on the second level, near Tim Hortons and the Ice Palace.
